If your child requires a tailored medical diet, you will be required to complete a digital application on the Derbyshire County Council Catering Service website. A link to the webpage can be found below.

Please follow the simple step by step instructions to create an account and upload a copy of medical documentation relating to your child’s condition and submit the digital application.
